UnitedHealthcare Chiropractor in The Woodlands, TX

We are in network with UnitedHealthcare. A step-by-step look at your first visit, from the call before you book to the statement you receive afterward.

By Dr. Mariam Mobarak, DC

Blue Zone Advanced Chiropractic is in network with UnitedHealthcare. Our office sits at the corner of Woodlands Parkway and FM 2978, in the Sterling Ridge area, a short drive from Magnolia, Tomball and Spring, and Dr. Mariam Mobarak, DC sees UnitedHealthcare members of all ages.

Instead of a list of insurance terms, this page follows your first visit from start to finish: what to do before you book, what happens while you are here, and what to expect afterward.

Before you book

Find your card

Your UnitedHealthcare card lists your member ID, group number and a phone number for member services. Keep it handy when you book, and make sure it is the newest one, since an old card can hold up a claim. If someone else in the family carries the coverage, you will need their name and date of birth as the policy holder.

Call us if you have questions

Call (281) 688-5580 with the card in front of you. The front desk can answer questions about how your visit is handled before you commit to anything.

Know how the $99 special fits in

We go with whichever costs you less, your insurance or the $99 special. The $99 covers your exam, X-rays and your first treatment. If your copay or deductible would cost you more than $99 for that first visit, the special is used. If your insurance would cost you less, we use your insurance.

Pick a time

Book online at any hour, or call during office hours. Office hours are Monday 10 a.m. to 7 p.m., Tuesday 9 a.m. to 6 p.m., Wednesday 10 a.m. to 7 p.m., Thursday 10 a.m. to 6 p.m. and Friday 9 a.m. to 2 p.m. We are closed on weekends.

While you are here

Check-in

Bring your UnitedHealthcare card, a photo ID and a list of your medications. If you have reports from an MRI or a specialist, bring those as well. Suite 600 is on the ground floor, so getting from the car to the front desk is easy even when your back is sore.

Exam and X-rays

The new patient visit is booked as a 30 minute appointment. You describe what hurts, when it started and what makes it better or worse. Then you have a physical and neurological exam, and X-rays are taken here in the office. X-rays are part of every new patient visit.

Talking through the findings

Dr. Mobarak explains what the exam and X-rays show in plain language, and you go over the options together. Care can include chiropractic adjustments using methods such as Diversified, Gonstead, Thompson, Activator and Upper Cervical, along with exercises to do at home.

After your visit

We send the claim

Because we are in network, our office files the claim with UnitedHealthcare. You do not need to submit anything yourself.

The statement in your mailbox

A few weeks later, UnitedHealthcare usually sends an Explanation of Benefits. It shows what was billed, what the insurer paid and what share is yours. It is a summary, not a bill. If something on it does not make sense, call us and we will go through it with you.

Follow-up visits

Follow-up visits are billed the same way, so the routine stays simple: show your card at check-in when anything has changed, and we handle the claim.

Questions between visits

If your pain changes, or a new symptom appears, call the office. Some symptoms, such as loss of bladder or bowel control or new weakness in a leg, need emergency medical care rather than a chiropractic visit, and we will tell you so.

A car accident is a different path

For injuries from a car accident, we bill your auto policy's personal injury protection (PIP) directly, and you pay nothing at the visit. Letters of protection are accepted when an attorney is involved. See our car accident injury page.

What UnitedHealthcare members see us for

Common reasons include back pain, neck pain, sciatica, knee pain, dizziness and vertigo and pregnancy-related discomfort. Children age three and up can have a primitive reflex screening.

Other insurance we accept

We accept insurance. We are in network with Medicare, UnitedHealthcare, Blue Cross Blue Shield, Cigna, ASH, Devoted, Aetna, Humana, Curative, Ambetter and Anthem.
